Services include Independent Living Instruction, Information & Referral Services, Peer Mentoring, Transition Services (Youth Transition, Transition out of Nursing Homes & Diversion), and Advocacy (includes self-advocacy and system advocacy). Has Peer Mentor groups, coalition groups, and social groups monthly and groups meet via telephone. Also provides Transportation Services to specific areas on the Navajo, Hopi, and San Juan tribal reservations. Provides Durable Medical Equipment loans.
