Eligibility

Coconino County residents age 60 and older for meals, adults with disabilities for in-home assistance. Congregate meals, meals on wheels, case management, and home care are only available in Flagstaff.

Description

Provides home care services to seniors including housekeeping, personal care, and respite. Also provides assistance to apply for Arizona Long Term Care System (ALTCS), Medicare Cost Share prescription assistance, Telephone Assistance Programs (TAP), and other benefits. Offers information about and referrals to community resources such as eviction prevention, clothing, food, health care, home repairs, hospice, elder rights and protection, senior centers, transportation, utility payment and discounts, volunteer opportunities, and widow/widower tax exemptions.
